MacKay Memorial Hospital Telemedicine Informed Consent Form

  1. I agree to receive self-pay telemedicine consultation services. These services include: (1) medical history review, (2) post-discharge follow-up care, (3) medication adjustment and care instructions, (4) health education, and (5) prescription services (available for rural and offshore island areas only).
  2. Service Fees:
    • For Overseas Taiwanese Nationals: NT$3,300 for 30 minutes of consultation. Additional NT$100 per minute for consultations exceeding 30 minutes.
    • For International Clients (Foreign Nationals): NT$7,500 for up to 30 minutes of cons. Additional NT$200 per minute for consultations exceeding 30 minutes.
  3. In accordance with the Medical Institution Medical Privacy Protection Regulations published by the Department of Medical Affairs, Ministry of Health and Welfare in 2015, audio and video recording requires patient consent.
  4. As required by the Department of Health, all telemedicine consultations must be recorded (audio and video) and archived to protect the rights and interests of both parties. Your rights are fully protected under applicable laws, including the confidentiality of personal information and privacy. Access to these records requires a formal medical record retrieval application.

  6. Legal Terms: Should this consent agreement prove insufficient in any respect, both parties shall negotiate in good faith in accordance with commercial practices and standard medical regulations. The legal relationships arising from this consent agreement shall be governed by and interpreted in accordance with the laws of the Republic of China, and principles of conflict of laws shall not apply. In the event of disputes or litigation arising from this agreement, both parties agree that the Taipei District Court shall serve as the court of first instance.
